The purpose of this report is to identify any safety-related issues that the organization may have. The methodology of the audit is not merely to identify the presence of compliance with regulatory requirements and mandatory documentation, but to establish if this compliance is actively engaged with and adhered to. This in line with the principles developed by Attis after years of research and real-world experiences. The auditing of safety is carried out under four headings: culture, operations, documentation, and administration (or management in this context), and is abbreviated as CODA. These categories are identified as the four ‘environments’ from which all types of risk to any organization can originate
